The dutch were nearly starved to death during Nazi occupation.  They were reduced to eating tulip bulbs.  Look up the "Hunger Winter."  And yet, a few generations later, they are significantly taller than American men, who suffered no such privations.

Anyway, women selecting mates is a relatively new phenomenon; the OP wrote "over millennia," but most mate selection during the last two thousand years was done by men, with women having very little choice in the matter. 

So maybe it's the other way around - maybe something unique to Dutch society has favored height for a very long time, leading Dutch men to select tall women when other cultures did not. 

If it was simply, "Dutch women select tall men," then the Dutch wouldn't be exceptional, as all western women prefer taller men.

They do not get the payout if there is non-compliance, so the drs ditch any patient who gets weird about shots trying to sabotage the ultimate reward every year.
The doctors don’t inject the vaccines themselves, there is an army of low-level injectors in the office who manage all shots & appts & records for them. The only role for the Dr is to pop in at the end, tell the illegal parents thru an interpreter that everything looks fantastic and they have the bestest & fattest baby ever, to keep them returning for more shots. When the anchor baby gets old enough - 8 years old or so, the illegal parents shove the kid on a plane alone to Guatemala to show off this spectacle of obesity to great grandma. The parents cannot go, but the American Citizen anchor baby gets a passport & plane ride for a month or so summer visit. The purpose isn’t really to show the anchor babies the homeland & have them experience hard life to teach them a lesson about their origins, have them experience culture & work hard for the summer - oh no. The only allowable scenario is for the kid to show up in the village as a rock star celebrity, rich beyond all measure, in designer clothes and shoes and bearing gifts and sporting enormous waddling girth. Sometimes parades & fireworks and village festivals are thrown for these rock star visiting anchor babies, with all festivities and gifts & cakes paid from by the illegal parents stuck in the US wiring obscene $$$$ through Western Union.
