Here are the 10 parts of my eldercare plan...
Below are the eldercare services that I will be using.
For each part, I've done my research, and figured out my best options.
- Geriatric Care Manager.
- Amazon Alexa smart speakers.
- Shower Chair.
- Bed Rail.
- Home Health Agency (this is different than a Home Care Agency)
- Assisted Living Facility.
- Hospitals
- Short-term Nursing Homes
- Long-term Nursing Homes
- Dignitas Suicide Clinic in Switzerland.

Eldercare Services that I will NOT be using:
- Independent Living / Senior Living Community. These are basically doorman apartment buildings, but you're paying extra for social activities & a cafeteria. I don't like socializing with old people, and I hate cafeterias (specific hours, different menu each day). In college, I absolutely hated the dorm cafeterias. I want to eat whenever I want, and whatever I want.
- Home Care Agency / Home Care Aide. They're just manual workers who can help you take a shower, etc. However, they need to be scheduled in 4 hr increments, and they generally don't show up on demand. So if I'm ever so disabled that I need help using the toilet & shower, then I need to be in an assisted living facility, with multiple call button options.

Not a bad list.
But if your plan is to remain in your primary residence, you may find that events have a different plan for you.

I've been managing my elderly mother's health decline for the past 20 years.
My conclusion is that when you are truly disabled, and you need constant help for basic tasks (using toilet, shower, can't even read anything anymore), you need on-demand help 24/7.
This can be done a few ways:
1) You live with multiple relatives, and at least one adult will be with you at any given time. If you are left alone for even one hour, you could end up shytting & pissing all over yourself.
2) You hire an around-the-clock 24/7 care team. In a high cost of living area, this could cost $400,000/yr. And these are just home care aides, not nurses & doctors.
3) You live in a good assisted living facility. In a high cost of living area, this could cost $70,000 - $140,000/yr, depending on the level of care needed. As with everything I listed in my original post, there are good ones vs bad ones. The good ones will have multiple call buttons, pull cords, pendants setup. They will have at least one nurse on staff to supervise medications. They will have an IT team to setup video calls with your doctors & relatives. They have a memory care program, and they have walking trails so that you don't have to intersect with cars & nogs.
Option #2 is expensive ($400,000/yr), and is also really weird, since you have people sitting on your couch 24/7, usually doing nothing.
Option #3 is far more reasonably priced ($70,000 - $140,000/yr), and services are far more comprehensive, since it's big campus full of specialists.

My estate plan is a work in progress.
By my conservative estimate:
- 10 years from now: Implement early parts of my eldercare plan (I'm fully functional, but getting ready for unexpected health events, and sudden loss of functions).
- 20 years from now: Implement latter parts of my eldercare plan (I need some help managing my "Activities of Daily Living").
- 30 years from now: Now I'm dead. Implement my estate plan.
These are conservative estimates. After all, I exercise every day, and I still have six-pack abs. It's possible that I live a longer healthy life.
